CPT 20552 | Trigger Point Injections (TPIs) | 1 Or 2 Muscle(s)

CPT code 20552 describes the procedure of injecting one or multiple trigger points in one or two muscles.

What Is CPT Code 20552?

CPT 20552 can be reported if a provider injects a single trigger point in one muscle or multiple trigger points in two muscles.

Trigger point injection (TPI) is a medical procedure to relieve pain caused by muscle knots or trigger points.

A trigger point is a tight area within a muscle that can be felt as a nodule or knot and can cause pain in the affected muscle and other areas of the body.

CPT 20552 only covers the procedure of injecting the trigger point(s) and does not include the cost of the medication or any other related services or supplies.

The CPT book describes CPT code 20552 as: “Injection(s); single or multiple trigger point(s), 1 or 2 muscle(s).”

The procedure begins with the patient being properly prepped for the injection. This may include cleaning the area, numbing it with a local anesthetic, and positioning the patient for the best access to the trigger point.

The healthcare provider will then perform a physical examination of the muscle in question, using their hands to palpate or touch the muscle to locate the trigger point.

Once the trigger point has been identified, the provider will apply firm pressure to the area to assess for referred pain and a twitch response.

After this, the provider will slowly inject the appropriate corticosteroid or anesthetic into the trigger point.

The corticosteroid will help to reduce inflammation and pain, while the anesthetic will numb the area and provide immediate pain relief. The procedure typically takes only a few minutes to complete.

TPI can treat various conditions such as chronic headaches, tension headaches, fibromyalgia, myofascial pain syndrome, and more.

How To Use CPT 20552

Use CPT code 20552 when the provider performs trigger point injections on one or two muscles, regardless of the number of injections.

If the provider performs trigger point injections on three or more muscles, you may use CPT code 20553 instead.

A patient comes in with pain in their neck and shoulders. The provider performs six injections on the patient’s trapezius muscle and three on the levator scapulae.

The correct code to report in this scenario is CPT 20552 because the provider performed TPIs on two muscles.
