Billing Guidelines
Reporting of CPT codes 33212, 33213, and 33221 should not be done together with CPT codes 33216 and 33217.
When removing and replacing a pacemaker pulse generator, it is not appropriate to report 33212, 33213, 33221, and 33233 together. Instead, use 33227-33229 as appropriate for pulse generator replacement.
If both the insertion of a generator and epicardial lead placement are done, it is necessary to report CPT codes 33202, 33203 along with 33212, 33213, and 33221.
