The CPT codes for Rotavirus vaccines are CPT 90680 and CPT 90681. For a three-dose schedule, you may use 90680, and for a two dose schedule, you can report CPT code 90681.

Descriptions

The official description of CPT 90680 is: “Rotavirus vaccine, pentavalent (RV5), 3 dose schedule, live, for oral use.”

The official description of CPT 90681 is: “Rotavirus vaccine, human, attenuated (RV1), 2 dose schedule, live, for oral use.”

Billing Guidelines

You may report CPT code 90680 if a three-dose schedule can be used for the oral use of a live pentavalent vaccine containing the actual virus.

The 90681 CPT code on the other hand contains a live attenuated human rotavirus vaccine and is also administered orally but in a two-dose schedule.

The minimum age for the Rotavirus vaccine is six weeks. The first dose can be administered between 6 and 12 weeks. The physician should not start the vaccine series later than the age of 12 weeks. Final doses of the Rotavirus vaccine should be administered before the baby is 32 weeks. The vaccine should not be administered afterward.
