CPT 99411 Description: CPT 99411 may be used when the provider offers advice for about 30 minutes on improving and maintaining health to a group of patients.

CPT Code 99412

CPT 99412 Description: CPT 99411 may be used when the provider offers advice for about 60 minutes on improving and maintaining health to a group of patients.

CPT Code 99415

CPT 99415 Description: CPT 99415 may be reported for the first 60 min of extra time clinical staff spends under the provider’s supervision.

The staff spends extra time with the patient (directly) on an outpatient E&M service or an office.

CPT Code 99416

CPT 99416 Description: CPT 99416 may be reported for every extra 30 min. spend by the staff spends under the supervision of the provider.

The staff spends extra time with the patient (directly) on an outpatient E&M service or an office.

CPT 99417 Description: Report CPT 99417 for every additional 15 min. beyond the minimum required time for CPT 99205 or CPT 99215.
