CPT 99487 Description: CPT 99487 may be used for the first hour (per calendar month) of provider–directed clinical staff time.
The patient has 2 (or more) chronic conditions of significant risk of decompensation, acute exacerbation, death, or functional decline. The provider supports and coordinates the clinical staff to provide patient care across multiple disciplines.
CPT 99489 Description: CPT 99489 is an add-on code for CPT 99487.
CPT 99487 may be used for every extra 30 minutes beyond the first hour (per calendar month) of provider–directed clinical staff time.
The patient has 2 (or more) chronic conditions of significant risk of decompensation, acute exacerbation, death, or functional decline. The provider supports and coordinates the clinical staff to provide patient care across multiple disciplines.
