HIM Technician

Support health information management processes within Albany Med Health System.

This entry-level role focuses on retrieving, preparing, scanning, and indexing clinical documentation for inpatient and outpatient facilities. The position helps maintain compliance with legal, regulatory, and organizational standards while protecting the integrity and confidentiality of patient information.

Essential duties

  • Ensure clinical documentation is available to coding/billing and clinical staff via the electronic health record within 24 hours of discharge.
  • Retrieve and sort clinical documentation from departments as directed by HIM leadership.
  • Prepare documentation for scanning by reconciling encounters, orders, referrals, and related records.
  • Prep, scan, and index incoming clinical documentation from internal providers and external referrers.
  • Review incoming documents received via mail, fax, and email, then route them to the appropriate departments.
  • Identify documents that constitute the legal content of the patient's health record in both electronic and paper formats.
  • Assign appropriate document types and supporting identifiers such as date of service.
  • Assist with collection and reporting of quality metrics related to PCMH and CMS quality measures.
  • Route documentation to physicians, nurses, referral teams, or other departments as appropriate.
  • Complete destruction of records after prep, scan, index, and QA processing is finished.
  • Support retention, storage, and purging activities according to departmental schedules.
  • Meet productivity and quality benchmarks established by HIM leadership.
  • Collaborate with the team to improve data accuracy and medical record integrity.
  • Report workflow or build issues to HIM leadership promptly.
  • Support other tasks and projects assigned by the HIM leadership team.
  • Recognize and report active or potential security risks to IT staff or a supervisor.
Requirements & Qualifications

Required qualifications

  • High school diploma or GED.
  • Or an associate degree in HIM with at least 80 hours of practical HIM experience in a hospital setting in lieu of work experience.
  • Familiarity with EHR systems, ideally Epic and OnBase.
  • Basic understanding of Patient-Centered Medical Home (PCMH) and CMS quality measures.
  • Strong organizational and prioritization skills.
  • Ability to multitask while remaining professional, focused, composed, and positive.
  • Excellent customer service skills.
  • Ability to build effective rapport with patients, coworkers, and medical staff.
  • Strong written and verbal communication skills.
  • Ability to follow direction and work independently.
  • Flexibility and willingness to take initiative.
  • Basic computer skills, including Excel, Word, and Microsoft Teams.
  • Ability to read and understand English.
  • Ability to maintain confidentiality and communicate with personnel at all levels.

Preferred qualifications

  • RHIA or RHIT credential.
