The HIM Technician supports health information management processes for inpatient and outpatient facilities within Albany Med Health System. This entry-level role focuses on retrieving, preparing, scanning, and indexing clinical documentation while maintaining the integrity, confidentiality, and compliance of patient information.
Key Responsibilities
- Ensure clinical documentation is available to coding, billing, and clinical staff in the electronic health record within 24 hours of discharge.
- Retrieve and sort clinical documentation from departments as directed by HIM leadership.
- Prepare records for scanning by reconciling encounters, orders, referrals, and related documentation.
- Prep, scan, and index incoming clinical documentation from internal and external sources.
- Review incoming documents by mail, fax, and email and route them to the appropriate department.
- Identify legal health record content in electronic and paper formats.
- Assign appropriate document types and supporting identifiers such as date of service.
- Support collection and reporting of quality metrics related to PCMH and CMS measures.
- Route incoming documentation to physicians, nurses, referral teams, or other departments as needed.
- Complete record destruction, storage, retention, and purging tasks according to schedule and policy.
- Meet productivity and quality benchmarks established by HIM leadership.
- Help improve data accuracy and integrity of medical records.
- Report workflow or system issues to HIM leadership.
- Follow security practices for sensitive and protected health information.
Requirements & Qualifications
Qualifications
- High school diploma or GED required.
- Associate degree in HIM with at least 80 hours of practical HIM hospital experience may be considered in lieu of work experience.
- Familiarity with EHR systems, ideally EPIC and OnBase.
- Basic understanding of Patient-Centered Medical Home (PCMH) and CMS quality measures.
- Strong organizational and time management skills.
