Medical Records Clerk - Scanner

Slocum-Dickson Medical Group

Responsible for scanning and indexing documents into the electronic health record. Also responsible for filing and retrieving paper documents and medical records as needed. Performs other duties as assigned by the HIM Manager and/or HIS Supervisor.

Duties & Responsibilities

  • Perform workflows associated with medical record document scanning and indexing.
  • Perform document correction processes when indicated.
  • Ensure accuracy by reviewing patient names and medical record numbers on all medical records and documents.
  • Prepare items to be scanned into the EMR and ensure required medical record numbers and provider initials are present.
  • Review documents for provider signatures and route unsigned documents through the provider acknowledgment workflow.
  • Use a document scanner and computer to index documents appropriately into the electronic medical record.
  • Locate patient names, medical record numbers, and information using the computer.
  • Pull medical records for providers as needed.
  • Incorporate loose miscellaneous reports into the paper medical record as needed.
  • File medical records in terminal digit order into the permanent file.
  • Affix loose reports into the paper medical record when filing or when necessary.
  • Use proper out-guide procedures for medical records removed from files.
  • Keep medical record files neat and orderly.
  • Assist with locating misplaced medical records.
  • Answer phone requests regarding medical records and related concerns.
  • Assemble and format paper medical records according to policy and procedure when required.
  • Assist with testing related to upgrades and system changes in the electronic health record.
  • Notify the HIM Manager and/or HIS Supervisor of problems related to patient medical records.
  • Assist with other duties as designated.
  • Request supplies as needed.
  • Work overtime when necessary.
  • Uphold confidentiality policies of the Health Information Management Department and SDMG.
  • Adhere to SDMG policies and procedures.
  • Attend required in-service programs and employee informational meetings.
  • Maintain productivity, time management, and attendance standards.
  • Follow incident reporting, safety, emergency preparedness, and OSHA requirements.
Requirements & Qualifications
  • High school diploma, equivalency, and/or experience.
  • Ability to stoop, bend, and reach.
  • Ability to lift at or above five pounds.
  • Ability to alphabetize.
  • Ability to stand on feet for long periods of time.
  • Ability to sit, stand, bend, and reach as required.
  • May require lifting up to 20 pounds.
  • Manual dexterity sufficient to operate standard office equipment such as a computer, scanner, fax, calculator, and telephone.
  • Normal hearing and vision.
