Under immediate supervision, this position maintains patient files and statistics, responds to requests for medical records, and performs clerical duties.
Key responsibilities include:
- Maintains department policies, procedures, quality improvement, safety, environmental, and infection control standards
- Delivers and retrieves patient charts from in-house clinics and practitioners
- Establishes and maintains inpatient and outpatient files and records
- Performs discharge analysis of inpatient, outpatient, and Same Day Surgery charts and notes chart deficiencies
- Updates and disseminates the deficiency list to physicians and allied health professionals
- Answers telephone calls and provides routine information
- Releases information to patients, physicians, attorneys, hospitals, medical facilities, and insurance companies under the direction of the Medical Records Director
- Maintains patient confidentiality
- Retrieves old medical record numbers for microfilmed MPI cards when required
- Pulls records for off-site scanning on a regular basis
- Checks permanent files for misfiles to ensure accurate filing
- Scans documents into the electronic medical record
- Performs chart audits for accurate charges and helps ensure timely charge updates to patient accounts
- Assists with care and maintenance of department equipment and supplies
- Participates in educational programs, in-services, and meetings
- Maintains positive and effective relationships with coworkers, other departments, patients, and visitors
- Performs other related duties as assigned
Requirements & Qualifications
Minimum qualifications:
- High school diploma or equivalent
- Preferred education in Health Information Management
- At least 1 year of experience working as a Medical Records Clerk in a medical records department in an acute care hospital
- Computer skills and strong people skills
- Ability to perform physical requirements including walking, lifting, reaching, hand-eye coordination, and speaking
- No certification, registration, or licensure required
