Responsible for collecting, tracking, and reconciling patient medical record information obtained from departments or nursing units after patient discharge. Serves as an information resource by responding to requests for general and medical record information received via phone, email, fax, or in person. Retrieves records for patient care, studies, committees, and other requests, and collaborates with other departments as needed.
Essential responsibilities
- Collect, track, and reconcile patient medical record information obtained after discharge.
- Batch, prep, scan, quality check, validate, and store records for document imaging and storage.
- Assist with physician chart completion and suspension processes, including assigning and validating chart deficiencies.
- Communicate with providers regarding incomplete records and assist with distribution of reports, letters, and statistics related to delinquent medical records.
- Escalate documentation holds to clinical and administrative personnel.
- Track tasks and time spent while meeting quality and production targets.
- Perform standardized chart audits following established guidelines and procedures.
- Use the EMR system to support record completion, coding, release of information, and maintenance of the legal health record.
- Perform EMPI tasks according to established policies, procedures, and timeframes.
- Assist with patient portal enrollment.
- Perform medical transcription tasks according to established policies, procedures, and timeframes.
Requirements & Qualifications
Required
- High school diploma
- 2 years of HIM experience
Preferred
- RHIT certified or eligible
- CRCR certified
- Associate's degree in HIM or another related healthcare field
