Support primary care providers with real-time clinical documentation so they can focus on patient care.
Responsibilities include:
- Join the provider in the exam room to observe patient visits and document encounters in real time
- Capture history of present illness, assessment, plan, and physical exam in structured notes
- Assign appropriate CPT and ICD-10 codes
- Prepare after-visit summaries
- Work with the provider to ensure documentation is accurate and specific
- Request and review medical records to support clinical documentation improvement
- Use population health tools to identify documentation opportunities and close care gaps
- Support daily huddles and clinical documentation reviews
- Provide administrative support such as placing orders, referrals, and addressing tasks
- Perform other duties as assigned
Requirements & Qualifications
Knowledge and qualifications include:
- Knowledge of medical terminology and common medications, gained through a pre-medical degree or prior clinical experience
- Prior clinical experience, including shadowing and/or volunteering, strongly preferred
- Prior scribe or transcription experience preferred but not required
- Advanced listening and communication skills
- Strong computer literacy and ability to learn new technical workflows
- Fluency in Spanish, Polish, Russian, or other community languages when required
- Ability to adapt quickly to new workflows and learn new concepts
- Typing speed of 70+ words per minute strongly preferred
- Ability to take direction and work as part of a patient care team
- Ability to work in person in clinics
- Commitment to at least 1 year in role, with 2+ years ideal
- Ability to work approximately 40-45 hours per week during clinic hours
- Compliance with HIPAA and other hospital and Oak Street Health policies
- US work authorization required
