Assist in maintaining residents’ medical records in accordance with facility policies and state and federal regulations, and serve as the designated Compliance Liaison for the facility.
Health Information Oversight
- Manage all aspects of medical records throughout their lifecycle, including admission through discharge and long-term retention where applicable.
- Ensure records are accurate, timely, complete, secure, and compliant with regulatory requirements.
- Upload paper-based patient records to the electronic chart promptly upon receipt.
- Maintain organized auxiliary clinical and administrative records in accordance with company retention policy.
- Manage long-term storage of paper documents, including labeling, preparing, and sending files to offsite storage.
- Assist with locating records housed in long-term storage and placing retrieval orders as needed.
- Follow up with hospitals and external providers to obtain documentation after transfers or appointments.
- Track physician visits to ensure regulatory compliance and obtain missing visit notes.
- Conduct routine audits to verify required documentation and present findings in daily morning meetings.
- Coordinate record requests by forwarding them to facility management within 24 hours and communicating with requestors as needed.
- Respond to approved medical record requests in accordance with company policy and confidentiality standards.
- Serve as a subject matter expert on health information management standards and regulations.
Compliance Liaison Duties
- Act as the facility Compliance Liaison.
- Support annual compliance risk assessments by reminding stakeholders to complete surveys.
- Support completion of annual compliance education by reminding stakeholders of due dates.
- Help distribute the code of conduct and compliance policies periodically.
- Help distribute monthly compliance newsletters and tips.
- Escalate compliance violations to the compliance office as appropriate.
- Serve as a communication channel between the facility and the compliance office.
Requirements & Qualifications
Qualifications
- Associate degree or higher in Health Information Management, Healthcare Administration, or a related field from an accredited college required; extensive Health Information experience may be considered in lieu of a degree.
- RHIT or RHIA credential preferred.
- Minimum 2 years of experience in a healthcare facility required.
- Skilled nursing facility or long-term care Health Information Management experience preferred.
- Knowledge of legal, ethical, and professional practice standards in Health Information Management.
- Knowledge of medical terminology, anatomy and physiology, and regulatory requirements.
- Strong analytical, critical thinking, and problem-solving skills.
- Intermediate to advanced skills in Microsoft Word, Excel, PowerPoint, and Outlook.
- Experience with EMR and/or MatrixCare preferred.
- Ability to exercise independent judgment, maintain confidentiality, and demonstrate professionalism and integrity.
