Focused Post Acute Care Partners is seeking an MDS Coordinator to join its skilled nursing team in Clarksville, TX.
The MDS Coordinator supports the Director of Nursing by ensuring documentation meets federal, state, and certification guidelines. This role coordinates the RAI process and helps ensure the timeliness and completeness of MDS, CAAs, and interdisciplinary care plans.
Key responsibilities include:
- Coordinating the RAI process and MDS assessments
- Supporting compliance with Medicare PPS, Medicaid reimbursement, OBRA, and state/federal regulations
- Ensuring accurate documentation and timely submission of resident assessment data
- Monitoring care plans, validation records, audits, and survey readiness
- Assisting with billing compliance, triple check meetings, and Medicaid payment processes
- Using electronic medical record systems, including PointClickCare
Requirements & Qualifications
Applicants should have:
- A current LVN license
- At least 2 years of nursing experience in a skilled nursing facility preferred
- Strong knowledge of the RAI process
- Familiarity with Medicare PPS and Medicaid reimbursement
- Understanding of Quality Indicator processes
- Knowledge of OBRA regulations and the Minimum Data Set (MDS)
- Understanding of the care plan process
- Ability to maintain accurate documentation and compliance standards
