Common Language Description

Quantitative sensory testing (QST) is a diagnostic procedure that evaluates the sensory response of a patient's extremities by applying various stimuli. This testing is crucial for assessing the integrity of sensory pathways and can help identify conditions affecting sensation. During the procedure, a specialized device is utilized to deliver specific types of sensory stimulation, which may include pressure, vibration, temperature, or pain. The physician interprets the responses from the patient to determine the functionality of the sensory nerves in the tested limb. It is important to note that if the testing involves specific stimuli such as pressure, vibration, cool temperatures, or heat and/or pain, there are designated CPT® codes (0106T, 0107T, 0108T, and 0109T, respectively) that should be used. However, when other types of stimuli not covered by these codes are employed for sensation testing, CPT® code 0110T is applicable. This code encompasses a broader range of sensory assessments, allowing for a comprehensive evaluation of the patient's sensory capabilities.

1. Indications

The quantitative sensory testing (QST) procedure is indicated for various clinical scenarios where assessment of sensory function is necessary. The following conditions may warrant the use of this testing:

  • Neuropathy Assessment of sensory nerve function in patients with suspected peripheral neuropathy, which may be due to diabetes, alcohol use, or other underlying conditions.
  • Neurological Disorders Evaluation of sensory pathways in patients with neurological disorders such as multiple sclerosis or stroke.
  • Chronic Pain Syndromes Investigation of sensory abnormalities in patients experiencing chronic pain syndromes, where altered sensation may be a contributing factor.
  • Post-Surgical Evaluation Monitoring sensory recovery in patients following surgical procedures that may impact nerve function.

2. Procedure

The quantitative sensory testing procedure involves several key steps to ensure accurate assessment of sensory responses. Each step is critical for the effective execution of the test:

  • Step 1: Patient Preparation The patient is positioned comfortably, and the limb to be tested is exposed. The physician explains the procedure to the patient, ensuring they understand what to expect during the testing process.
  • Step 2: Selection of Stimuli The physician selects the appropriate sensory stimuli based on the specific aspects of sensation to be tested. This may include pressure, vibration, temperature, or other stimuli not covered by existing codes.
  • Step 3: Application of Stimuli The device is used to apply the selected stimuli to the patient's limb. The intensity and duration of the stimuli are carefully controlled to elicit a measurable sensory response.
  • Step 4: Response Measurement The physician observes and records the patient's responses to the stimuli. This may involve asking the patient to indicate when they feel the sensation or measuring the threshold at which the sensation is perceived.
  • Step 5: Interpretation of Results After completing the testing, the physician interprets the results to assess the sensory function of the tested limb. This interpretation is crucial for diagnosing any underlying conditions affecting sensory pathways.

3. Post-Procedure

Following the quantitative sensory testing, the physician may provide the patient with information regarding the results and any necessary follow-up actions. Depending on the findings, further diagnostic testing or referrals to specialists may be recommended. Patients are typically advised on any post-procedure care, which may include monitoring for any unusual symptoms or changes in sensation. The physician may also discuss potential treatment options based on the test results, particularly if abnormalities in sensory function are identified.
