Common Language Description

Secondary closure of an extensive or complicated surgical wound or wound dehiscence refers to a surgical procedure aimed at closing wounds that either were not closed during the initial surgery or have reopened along the original suture line. This procedure is necessary when a surgical wound has not healed properly, leading to complications that require further intervention. The term "secondary closure" indicates that this is a follow-up procedure, distinct from the original surgical operation, and is performed in a separate surgical session. During this process, the edges of the wound are meticulously trimmed to promote optimal healing. The closure technique may involve the use of absorbable sutures for deeper layers, with the knots buried to minimize irritation, while non-absorbable sutures are typically used for the superficial layers. In some cases, retention sutures may be employed to secure the wound edges without applying tension, utilizing tubing to facilitate this process. Additionally, stents may be utilized to maintain the position of the tissue or to keep an orifice open. Careful alignment of the wound edges is crucial to prevent complications such as scar depression. 1. Indications

The procedure of secondary closure of surgical wounds or dehiscence is indicated in the following scenarios:

  • Extensive Surgical Wound This procedure is performed when a surgical wound is not closed at the time of the original surgical procedure, necessitating a follow-up intervention to promote healing.
  • Wound Dehiscence This procedure is indicated when a previously closed surgical wound has opened along the suture line, requiring evaluation and repair to restore integrity and function.

2. Procedure

The procedure for secondary closure of a surgical wound or dehiscence involves several critical steps to ensure effective healing and minimize complications.

  • Step 1: Evaluation of the Wound The first step involves a thorough evaluation of the wound to assess the extent of the dehiscence or the condition of the extensive surgical wound. This assessment is crucial for determining the appropriate course of action.
  • Step 2: Wound Irrigation The wound is then irrigated with sterile saline or an antibiotic solution to cleanse the area and reduce the risk of infection. This step is essential for preparing the wound for closure.
  • Step 3: Trimming the Edges After irrigation, the previously placed sutures are removed, and the edges of the wound are carefully trimmed. This trimming helps to create a clean edge for optimal healing.
  • Step 4: Debridement of Necrotic Tissue Any necrotic tissue present in the wound is debrided to promote healing and prevent infection. This step is vital for ensuring that only healthy tissue is involved in the closure.
  • Step 5: Closure of Deep Layers The deepest layers of the wound may be closed using absorbable sutures, with the knots buried to minimize irritation to the surrounding tissue.
  • Step 6: Closure of Superficial Layers The superficial layers of the wound are then closed with non-absorbable sutures, ensuring that the wound edges are aligned properly to facilitate healing.
  • Step 7: Use of Retention Sutures If necessary, retention sutures may be placed through the entire thickness of the wound to hold the edges together without tension. A short length of plastic or rubber tubing is threaded over each suture, and each suture is tied to secure the closure.
  • Step 8: Application of Stents Stents may also be applied to hold tissue in place or maintain the opening of an orifice, depending on the specific requirements of the wound.

3. Post-Procedure

Post-procedure care involves monitoring the wound for signs of infection and ensuring that the closure remains intact. Patients may be advised on wound care practices, including keeping the area clean and dry. Follow-up appointments are typically scheduled to assess healing progress and to remove any non-absorbable sutures if applicable. 78 Unplanned return to the operating/procedure room by the same physician or other qualified health care professional following initial procedure for a related procedure during the postoperative period: it may be necessary to indicate that another procedure was performed during the postoperative period of the initial procedure (unplanned procedure following initial procedure). when this procedure is related to the first, and requires the use of an operating/procedure room, it may be reported by adding modifier 78 to the related procedure. (for repeat procedures, see modifier 76.)

59 Distinct procedural service: under certain circumstances, it may be necessary to indicate that a procedure or service was distinct or independent from other non-e/m services performed on the same day. modifier 59 is used to identify procedures/services, other than e/m services, that are not normally reported together, but are appropriate under the circumstances. documentation must support a different session, different procedure or surgery, different site or organ system, separate incision/excision, separate lesion, or separate injury (or area of injury in extensive injuries) not ordinarily encountered or performed on the same day by the same individual. however, when another already established modifier is appropriate it should be used rather than modifier 59. only if no more descriptive modifier is available, and the use of modifier 59 best explains the circumstances, should modifier 59 be used. note: modifier 59 should not be appended to an e/m service. to report a separate and distinct e/m service with a non-e/m service performed on the same date, see modifier 25.
51 Multiple procedures: when multiple procedures, other than e/m services, physical medicine and rehabilitation services or provision of supplies (eg, vaccines), are performed at the same session by the same individual, the primary procedure or service may be reported as listed. the additional procedure(s) or service(s) may be identified by appending modifier 51 to the additional procedure or service code(s). note: this modifier should not be appended to designated "add-on" codes (see appendix d).

79 Unrelated procedure or service by the same physician or other qualified health care professional during the postoperative period: the individual may need to indicate that the performance of a procedure or service during the postoperative period was unrelated to the original procedure. this circumstance may be reported by using modifier 79. 82 Assistant surgeon (when qualified resident surgeon not available): the unavailability of a qualified resident surgeon is a prerequisite for use of modifier 82 appended to the usual procedure code number(s).
22 Increased procedural services: when the work required to provide a service is substantially greater than typically required, it may be identified by adding modifier 22 to the usual procedure code. documentation must support the substantial additional work and the reason for the additional work (ie, increased intensity, time, technical difficulty of procedure, severity of patient's condition, physical and mental effort required). note: this modifier should not be appended to an e/m service.
32 Mandated services: services related to mandated consultation and/or related services (eg, third party payer, governmental, legislative or regulatory requirement) may be identified by adding modifier 32 to the basic procedure.
52 Reduced services: under certain circumstances a service or procedure is partially reduced or eliminated at the discretion of the physician or other qualified health care professional. under these circumstances the service provided can be identified by its usual procedure number and the addition of modifier 52, signifying that the service is reduced. this provides a means of reporting reduced services without disturbing the identification of the basic service. note: for hospital outpatient reporting of a previously scheduled procedure/service that is partially reduced or cancelled as a result of extenuating circumstances or those that threaten the well-being of the patient prior to or after administration of anesthesia, see modifiers 73 and 74 (see modifiers approved for asc hospital outpatient use).
53 Discontinued procedure: under certain circumstances, the physician or other qualified health care professional may elect to terminate a surgical or diagnostic procedure. due to extenuating circumstances or those that threaten the well being of the patient, it may be necessary to indicate that a surgical or diagnostic procedure was started but discontinued. this circumstance may be reported by adding modifier 53 to the code reported by the individual for the discontinued procedure. note: this modifier is not used to report the elective cancellation of a procedure prior to the patient's anesthesia induction and/or surgical preparation in the operating suite. for outpatient hospital/ambulatory surgery center (asc) reporting of a previously scheduled procedure/service that is partially reduced or cancelled as a result of extenuating circumstances or those that threaten the well being of the patient prior to or after administration of anesthesia, see modifiers 73 and 74 (see modifiers approved for asc hospital outpatient use).
54 Surgical care only: when 1 physician or other qualified health care professional performs a surgical procedure and another provides preoperative and/or postoperative management, surgical services may be identified by adding modifier 54 to the usual procedure number.
76 Repeat procedure or service by same physician or other qualified health care professional: it may be necessary to indicate that a procedure or service was repeated by the same physician or other qualified health care professional subsequent to the original procedure or service. this circumstance may be reported by adding modifier 76 to the repeated procedure or service. note: this modifier should not be appended to an e/m service.
77 Repeat procedure by another physician or other qualified health care professional: it may be necessary to indicate that a basic procedure or service was repeated by another physician or other qualified health care professional subsequent to the original procedure or service. this circumstance may be reported by adding modifier 77 to the repeated procedure or service. note: this modifier should not be appended to an e/m service.
