1. Indications

Radiologic examination of the ankle through arthrography is indicated for various clinical scenarios where detailed visualization of the ankle joint is necessary. The following conditions may warrant this procedure:

  • Joint Pain Persistent or unexplained pain in the ankle joint that may require further investigation to determine the underlying cause.
  • Joint Instability Suspected instability of the ankle joint, which may be due to ligament injuries or other structural abnormalities.
  • Intra-articular Pathology Evaluation of potential intra-articular lesions, such as tears in the cartilage or ligaments, that may not be visible on standard radiographs.
  • Preoperative Assessment Assessment of the ankle joint prior to surgical intervention to ensure accurate planning and evaluation of the joint's condition.

2. Procedure

The procedure for ankle arthrography involves several key steps that ensure accurate imaging and effective use of the radiopaque contrast material. The following procedural steps are performed:

  • Step 1: Preparation The patient is positioned comfortably, and the skin over the injection site is thoroughly cleansed to reduce the risk of infection. A local anesthetic is then injected to minimize discomfort during the procedure.
  • Step 2: Aspiration A needle is carefully inserted into the ankle joint space. If there is any fluid present in the joint, it is aspirated using a syringe. This step is crucial as it helps to clear the joint space for optimal imaging.
  • Step 3: Injection of Contrast Material Following aspiration, a radiopaque contrast agent is injected into the joint. This substance is essential for enhancing the visibility of the joint structures during imaging.

3. Post-Procedure

59 Distinct procedural service: under certain circumstances, it may be necessary to indicate that a procedure or service was distinct or independent from other non-e/m services performed on the same day. modifier 59 is used to identify procedures/services, other than e/m services, that are not normally reported together, but are appropriate under the circumstances. documentation must support a different session, different procedure or surgery, different site or organ system, separate incision/excision, separate lesion, or separate injury (or area of injury in extensive injuries) not ordinarily encountered or performed on the same day by the same individual. however, when another already established modifier is appropriate it should be used rather than modifier 59. only if no more descriptive modifier is available, and the use of modifier 59 best explains the circumstances, should modifier 59 be used. note: modifier 59 should not be appended to an e/m service. to report a separate and distinct e/m service with a non-e/m service performed on the same date, see modifier 25.
