Common Language Description

A radiologic examination of the foot, designated by CPT® Code 73630, involves a comprehensive imaging process that captures detailed views of the bones in the distal lower extremity. This examination typically includes the tibia, fibula, talus, calcaneus, cuboid, navicular, cuneiform, metatarsals, and phalanges. The standard views captured during this examination include the dorsal plantar (DP) view, which is taken from top to bottom, the lateral view from the side, the oblique view, which involves positioning the body and leg in a semi-prone position with partial rotation, and a stress study that applies manual traction to the joint. It is important to note that CPT® Code 73630 is specifically used to report a complete X-ray examination of the foot that encompasses a minimum of three views, distinguishing it from CPT® Code 73620, which is applicable for an X-ray exam of the foot with only two views.

1. Indications

The radiologic examination of the foot, as represented by CPT® Code 73630, is indicated for various clinical scenarios. The following conditions or symptoms may warrant this imaging procedure:

  • Pain Persistent or acute pain in the foot that requires investigation to determine the underlying cause.
  • Limping Abnormal gait or limping that may suggest an injury or structural issue within the foot.
  • Swelling Unexplained swelling in the foot that could indicate trauma, infection, or other pathological conditions.
  • Fractures Suspected fractures that need confirmation through imaging to guide treatment decisions.
  • Dislocations Evaluation of potential dislocations that may require immediate intervention.
  • Deformities Assessment of congenital or acquired deformities of the foot that may affect function.
  • Degenerative disease Investigation of degenerative conditions such as arthritis that may impact the foot's structure and function.
  • Osteomyelitis Diagnosis of bone infections that may present with localized pain and swelling.
  • Foreign body Detection of foreign objects that may have penetrated the foot, causing pain or infection.
  • Cysts or tumors Identification of abnormal growths or lesions within the foot that require further evaluation.

2. Procedure

The procedure for a complete radiologic examination of the foot involves several key steps to ensure comprehensive imaging. The following procedural steps are typically followed:

  • Step 1: Patient Preparation The patient is positioned appropriately, usually standing or sitting, to facilitate optimal imaging angles. The foot being examined is often placed in a specific position to capture the necessary views.
  • Step 2: Initial Imaging The first set of images is taken, typically starting with the dorsal plantar (DP) view, which captures the foot from top to bottom. This view provides a clear representation of the foot's overall structure.
  • Step 3: Lateral Imaging The lateral view is then obtained, which involves taking an image from the side of the foot. This view is crucial for assessing the alignment and profile of the foot's bones.
  • Step 4: Oblique Imaging An oblique view is captured next, where the foot is positioned in a semi-prone manner with partial rotation. This view helps visualize the bones in a different orientation, revealing additional details that may not be apparent in the standard views.
  • Step 5: Stress Study (if indicated) If necessary, a stress study may be performed, which involves applying manual traction to the joint. This step is particularly useful for evaluating joint stability and alignment under stress conditions.
  • Step 6: Image Review After all necessary images are captured, the radiologist or technician reviews the images for clarity and completeness, ensuring that all required views are included in the study.

3. Post-Procedure

Following the radiologic examination of the foot, the patient may be advised on post-procedure care, although specific instructions may vary based on individual circumstances. Generally, there are no significant restrictions following the procedure, as it is non-invasive and does not require recovery time. Patients may resume normal activities unless otherwise directed by their healthcare provider. The images obtained will be interpreted by a radiologist, who will provide a report detailing the findings. This report is essential for guiding further clinical management, including potential referrals to specialists or recommendations for additional diagnostic tests if necessary.

59 Distinct procedural service: under certain circumstances, it may be necessary to indicate that a procedure or service was distinct or independent from other non-e/m services performed on the same day. modifier 59 is used to identify procedures/services, other than e/m services, that are not normally reported together, but are appropriate under the circumstances. documentation must support a different session, different procedure or surgery, different site or organ system, separate incision/excision, separate lesion, or separate injury (or area of injury in extensive injuries) not ordinarily encountered or performed on the same day by the same individual. however, when another already established modifier is appropriate it should be used rather than modifier 59. only if no more descriptive modifier is available, and the use of modifier 59 best explains the circumstances, should modifier 59 be used. note: modifier 59 should not be appended to an e/m service. to report a separate and distinct e/m service with a non-e/m service performed on the same date, see modifier 25.

25 Significant, separately identifiable evaluation and management service by the same physician or other qualified health care professional on the same day of the procedure or other service: it may be necessary to indicate that on the day a procedure or service identified by a cpt code was performed, the patient's condition required a significant, separately identifiable e/m service above and beyond the other service provided or beyond the usual preoperative and postoperative care associated with the procedure that was performed. a significant, separately identifiable e/m service is defined or substantiated by documentation that satisfies the relevant criteria for the respective e/m service to be reported (see evaluation and management services guidelines for instructions on determining level of e/m service). the e/m service may be prompted by the symptom or condition for which the procedure and/or service was provided. as such, different diagnoses are not required for reporting of the e/m services on the same date. this circumstance may be reported by adding modifier 25 to the appropriate level of e/m service. note: this modifier is not used to report an e/m service that resulted in a decision to perform surgery. see modifier 57 for significant, separately identifiable non-e/m services, see modifier 59.
27 Multiple outpatient hospital e/m encounters on the same date: for hospital outpatient reporting purposes, utilization of hospital resources related to separate and distinct e/m encounters performed in multiple outpatient hospital settings on the same date may be reported by adding modifier 27 to each appropriate level outpatient and/or emergency department e/m code(s). this modifier provides a means of reporting circumstances involving evaluation and management services provided by physician(s) in more than one (multiple) outpatient hospital setting(s) (eg, hospital emergency department, clinic). note: this modifier is not to be used for physician reporting of multiple e/m services performed by the same physician on the same date. for physician reporting of all outpatient evaluation and management services provided by the same physician on the same date and performed in multiple outpatient setting(s) (eg, hospital emergency department, clinic), see evaluation and management, emergency department, or preventive medicine services codes.
51 Multiple procedures: when multiple procedures, other than e/m services, physical medicine and rehabilitation services or provision of supplies (eg, vaccines), are performed at the same session by the same individual, the primary procedure or service may be reported as listed. the additional procedure(s) or service(s) may be identified by appending modifier 51 to the additional procedure or service code(s). note: this modifier should not be appended to designated "add-on" codes (see appendix d).
52 Reduced services: under certain circumstances a service or procedure is partially reduced or eliminated at the discretion of the physician or other qualified health care professional. under these circumstances the service provided can be identified by its usual procedure number and the addition of modifier 52, signifying that the service is reduced. this provides a means of reporting reduced services without disturbing the identification of the basic service. note: for hospital outpatient reporting of a previously scheduled procedure/service that is partially reduced or cancelled as a result of extenuating circumstances or those that threaten the well-being of the patient prior to or after administration of anesthesia, see modifiers 73 and 74 (see modifiers approved for asc hospital outpatient use).
53 Discontinued procedure: under certain circumstances, the physician or other qualified health care professional may elect to terminate a surgical or diagnostic procedure. due to extenuating circumstances or those that threaten the well being of the patient, it may be necessary to indicate that a surgical or diagnostic procedure was started but discontinued. this circumstance may be reported by adding modifier 53 to the code reported by the individual for the discontinued procedure. note: this modifier is not used to report the elective cancellation of a procedure prior to the patient's anesthesia induction and/or surgical preparation in the operating suite. for outpatient hospital/ambulatory surgery center (asc) reporting of a previously scheduled procedure/service that is partially reduced or cancelled as a result of extenuating circumstances or those that threaten the well being of the patient prior to or after administration of anesthesia, see modifiers 73 and 74 (see modifiers approved for asc hospital outpatient use).
55 Postoperative management only: when 1 physician or other qualified health care professional performed the postoperative management and another performed the surgical procedure, the postoperative component may be identified by adding modifier 55 to the usual procedure number.
73 Discontinued out-patient hospital/ambulatory surgery center (asc) procedure prior to the administration of anesthesia: due to extenuating circumstances or those that threaten the well being of the patient, the physician may cancel a surgical or diagnostic procedure subsequent to the patient's surgical preparation (including sedation when provided, and being taken to the room where the procedure is to be performed), but prior to the administration of anesthesia (local, regional block(s) or general). under these circumstances, the intended service that is prepared for but cancelled can be reported by its usual procedure number and the addition of modifier 73. note: the elective cancellation of a service prior to the administration of anesthesia and/or surgical preparation of the patient should not be reported. for physician reporting of a discontinued procedure, see modifier 53.
78 Unplanned return to the operating/procedure room by the same physician or other qualified health care professional following initial procedure for a related procedure during the postoperative period: it may be necessary to indicate that another procedure was performed during the postoperative period of the initial procedure (unplanned procedure following initial procedure). when this procedure is related to the first, and requires the use of an operating/procedure room, it may be reported by adding modifier 78 to the related procedure. (for repeat procedures, see modifier 76.)
95 Synchronous telemedicine service rendered via a real-time interactive audio and video telecommunications system: synchronous telemedicine service is defined as a real-time interaction between a physician or other qualified health care professional and a patient who is located at a distant site from the physician or other qualified health care professional. the totality of the communication of information exchanged between the physician or other qualified health care professional and the patient during the course of the synchronous telemedicine service must be of an amount and nature that would be sufficient to meet the key components and/or requirements of the same service when rendered via a face-to-face interaction. modifier 95 may only be appended to the services listed in appendix p. appendix p is the list of cpt codes for services that are typically performed face-to-face, but may be rendered via a real-time (synchronous) interactive audio and video telecommunications system.
99 Multiple modifiers: under certain circumstances 2 or more modifiers may be necessary to completely delineate a service. in such situations modifier 99 should be added to the basic procedure, and other applicable modifiers may be listed as part of the description of the service.
