Official Description

Computed tomography, lower extremity; with contrast material(s)

Common Language Description

Computed tomography (CT) of the lower extremity, specifically coded as CPT® 73701, is a diagnostic imaging procedure that utilizes advanced X-ray technology to create detailed images of the leg's tissues and bone structures. This procedure employs multiple, narrow X-ray beams that rotate around a single axis, capturing a series of two-dimensional (2D) images from various angles. The use of contrast material, typically an iodine-based dye administered intravenously, enhances the visibility of the targeted area, allowing for clearer and more precise imaging. The data collected during the scan is processed by computer software, which generates several thin, cross-sectional 2D slices of the leg. These individual slices can be stacked to create three-dimensional models, providing a comprehensive view of the anatomy. The patient lies on a table that moves into the CT scanner, where the imaging takes place. Unlike CPT® 73700, which does not utilize contrast material, CPT® 73701 specifically involves the use of contrast to improve the diagnostic quality of the images. This procedure is crucial for identifying various conditions, including tumors, abscesses, or masses, as well as assessing bone integrity for degenerative diseases, fractures, or injuries resulting from trauma. The physician interprets the CT images, documenting any abnormalities and providing a detailed written report of the findings.

1. Indications

Computed tomography of the lower extremity with contrast material (CPT® 73701) is indicated for various clinical scenarios where detailed imaging of the leg is necessary. The following conditions may warrant this procedure:

  • Suspected Tumors - When there is a need to locate and evaluate the presence of tumors within the leg tissues.
  • Abscesses or Masses - To identify and assess any abscesses or abnormal masses that may be causing symptoms.
  • Bone Evaluation - For evaluating the bones of the lower extremity for degenerative conditions, fractures, or other injuries, particularly following trauma.
  • Pain or Swelling Investigation - To determine the underlying cause of unexplained pain or swelling in the leg.

2. Procedure

The procedure for performing a computed tomography scan of the lower extremity with contrast material involves several key steps:

  • Patient Preparation - The patient is prepared for the procedure, which may include obtaining a medical history and ensuring that there are no contraindications to the use of contrast material, such as allergies to iodine.
  • Contrast Administration - An intravenous line is established, and iodine-based contrast dye is administered to enhance the imaging of the lower extremity. This step is crucial for improving the visibility of blood vessels and soft tissues during the scan.
  • Positioning - The patient is positioned on the CT scanner table, ensuring that the lower extremity is properly aligned for optimal imaging. The technician may provide instructions to the patient regarding breath-holding or remaining still during the scan.
  • Image Acquisition - The CT scanner rotates around the patient, capturing multiple 2D images of the lower extremity from various angles. The advanced technology allows for the creation of thin cross-sectional slices of the leg, which are then processed to form detailed images.
  • Image Review - After the imaging is complete, the physician reviews the CT scan images to identify any abnormalities. This includes looking for tumors, abscesses, or masses, as well as evaluating the condition of the bones for fractures or degenerative changes.
  • Report Generation - The physician provides a written interpretation of the findings, documenting any identified issues and recommendations for further action if necessary.

3. Post-Procedure

After the computed tomography scan with contrast material is completed, the patient may be monitored for a short period to ensure there are no adverse reactions to the contrast dye. It is important to provide the patient with post-procedure instructions, which may include hydration recommendations to help flush the contrast material from the body. The physician will discuss the results of the scan with the patient, explaining any findings and potential next steps in their care. Follow-up appointments may be scheduled to address any identified issues or to discuss further diagnostic or treatment options based on the CT findings.

RT Right side (used to identify procedures performed on the right side of the body)
LT Left side (used to identify procedures performed on the left side of the body)
50 Bilateral procedure: unless otherwise identified in the listings, bilateral procedures that are performed at the same session, should be identified by adding modifier 50 to the appropriate 5 digit code. note: this modifier should not be appended to designated "add-on" codes (see appendix d).
GC This service has been performed in part by a resident under the direction of a teaching physician
X5 Diagnostic services requested by another clinician: for reporting services by a clinician who furnishes care to the patient only as requested by another clinician or subsequent and related services requested by another clinician; this modifier is reported for patient relationships that may not be adequately captured by the above alternative categories; reporting clinician service examples include but are not limited to, the radiologist's interpretation of an imaging study requested by another clinician
51 Multiple procedures: when multiple procedures, other than e/m services, physical medicine and rehabilitation services or provision of supplies (eg, vaccines), are performed at the same session by the same individual, the primary procedure or service may be reported as listed. the additional procedure(s) or service(s) may be identified by appending modifier 51 to the additional procedure or service code(s). note: this modifier should not be appended to designated "add-on" codes (see appendix d).
TC Technical component; under certain circumstances, a charge may be made for the technical component alone; under those circumstances the technical component charge is identified by adding modifier 'tc' to the usual procedure number; technical component charges are institutional charges and not billed separately by physicians; however, portable x-ray suppliers only bill for technical component and should utilize modifier tc; the charge data from portable x-ray suppliers will then be used to build customary and prevailing profiles
X4 Episodic/focused services: for reporting services by clinicians who provide focused care on particular types of treatment limited to a defined period and circumstance; the patient has a problem, acute or chronic, that will be treated with surgery, radiation, or some other type of generally time-limited intervention; reporting clinician service examples include but are not limited to, the orthopedic surgeon performing a knee replacement and seeing the patient through the postoperative period
