Easterseals Blake Foundation provides comprehensive behavioral health services, child welfare, and support for individuals with intellectual and developmental disabilities across Southern Arizona. This full-time Behavioral Health Clinician / Case Manager role involves assessing behavioral health needs, coordinating care and services for adults and children eligible for public behavioral health services, conducting therapy and support services, crisis intervention, and extensive documentation in compliance with agency and funding requirements.
The position requires coordination with members, providers, and community agencies to ensure continuity of care, including scheduling, attending telemedicine appointments, and facilitating transportation and hospital discharge appointments. Additional responsibilities include rehabilitation services, case management, and participation in professional development and agency meetings.
The work environment includes office settings with interruptions, field visits to various sites and homes, and the ability to lift up to 50 lbs. Some weekend and evening hours are required, and additional hours may be necessary depending on circumstances.
Qualifications include a High School Diploma/GED with 1 year behavioral health experience or Associate’s/Bachelor’s degree in Behavioral Health. Bilingual Spanish is a plus. Must be 21+, hold an unrestricted AZ driver’s license with no major violations in the past 3 years, pass criminal background checks, and obtain AZ DPS Level 1 fingerprint clearance. CPR and First Aid certifications are provided by employer. Strong clinical knowledge of developmental stages, managed care systems, trauma-informed care, and diagnostic criteria (DSM-5-TR and DC-0-5) are essential.
The role demands a calm, supportive, self-directed professional with excellent interpersonal and computer skills capable of handling high workload and stressful situations.
