The Medical Coder II is responsible for accurately coding healthcare claims and analyzing denials to support proper reimbursement. This role codes hospital outpatient and professional services using official code sets and classification systems, reviews documentation across multiple EMR platforms, and works with clients to ensure coding accuracy.
Key responsibilities
- Read and analyze patient records
- Code services including evaluation and management, laboratory, imaging, injections and infusions, and specialty surgical procedures in clinic and hospital outpatient settings
- Monitor, research, and correct claim denials within health plan requirements and document follow-up trends
- Submit clean claims for payment
- Comply with federal and state standards, including CCI edits, Medicare bulletins, and ACR bulletins
- Maintain knowledge of coding guidelines and industry changes
- Locate documentation in EMR systems such as Epic, eCW, Cerner, and Meditech
- Interact with clients to ensure accuracy
- Maintain patient confidentiality and information security
- Maintain an error rate of 5% or less
- Meet production goals assigned by the supervisor
Requirements & Qualifications
Required qualifications
- High school diploma or equivalent
- 5 years of on-the-job experience in abstract coding and coding denials for hospital outpatient and professional claims
- Payor and policy research experience
- Experience with the Epic platform
- Current certification from AAPC or AHIMA, such as:
- RHIA
- RHIT
- CCS
- CPC
- Or equivalent certification from AAPC or AHIMA
Preferred qualifications
- Associate degree or equivalent in Health Information Management
- Meditech experience
- Rural Health Clinic experience
- Critical Access Healthcare experience
