Provides timely and accurate administrative and clinical data through the accurate assignment of current ICD-10-CM/PCS, CPT, or HCPCS codes while complying with federal, state, and hospital policies and procedures. Supports the management planning process and helps ensure appropriate reimbursement for services.
Performs accurate coding for inpatient and outpatient record types, including emergency, same day surgery, observation, pain clinic, wound clinic, diagnostics, and recurring accounts as applicable. Uses coding applications such as 3M 360, CAC, and Epic to support coding accuracy.
Ensures that medical record documentation supports accurate coding and reimbursement, requests clarification when information is incomplete, and assists physicians and other hospital personnel with coding and DRG/APC questions. Reviews challenged charts returned by payers and maintains institutional medical record policies and procedures.
Maintains high coding accuracy and productivity standards while keeping current with coding conventions, guidelines, and reimbursement methodologies.
Minimum Qualifications
- Completion of a 2-year college or technical school curriculum in Health Information Management, or an equivalent combination of relevant education and experience
- Preferred: completion of a 4-year college curriculum in Health Information Management
- 5 years of acute care hospital coding experience required
- One of the following certifications required: RHIT, RHIA, CCS, CCS-P, CPC, CIC, or COC
Knowledge and Skills
- Knowledge of current ICD-10-CM and ICD-10-PCS codes
- Knowledge of APC reimbursement models, UHDDS sequencing, and DRG payment methodologies, including MS-DRGs and APR-DRGs
- Knowledge of medical terminology
- Ability to code medical information accurately and maintain databases
- Ability to organize work to meet strict deadlines
- Strong oral and written communication skills
- Ability to read and interpret professional journals, governmental regulations, and coding guidelines
- Ability to follow written and verbal instructions
