Codes and abstracts hospital medical records for diagnostic and procedural coding across inpatient, observation, outpatient surgery, invasive outpatient, and emergency department settings.
Uses federal and state coding guidelines to maintain accuracy, abstracting quality, and productivity standards.
Collaborates with medical staff and CDI teams to clarify documentation.
Maintains compliance with corporate requirements related to medical record coding, abstracting, and DRG assignment.
Reviews records using ICD-10-CM, ICD-10-PCS, CPT-4, and encoder tools to code diagnoses and procedures according to AHA, AHIMA, UHDDS, and facility-specific rules.
Sequences principal diagnoses and procedures based on documentation and UHDDS definitions.
Validates DRG assignments and abstracts required data into the computer system.
Participates in maintaining DNB and accounts receivable goals, department competencies, and performance improvement activities.
Minimum of 2 years of inpatient records coding experience or equivalent.
High school diploma or GED required.
Knowledge of anatomy and physiology and medical terminology required.
Coding education preferred, or equivalent experience.
AHIMA Certified Coding Specialist (CCS) required for employees hired after 10/1/2025.
Ability to work in a Microsoft Windows environment.

Previous experience with an electronic legal health record system preferred.
