Horizon Home Care & Hospice is seeking a Pool Medical Records Coder to support home care and hospice operations. This role is located at the Milwaukee-based business office and includes work from home after orientation. The position is essential to accurate medical coding and optimal reimbursement.
The coder will work with physicians, clinicians, and staff while maintaining professionalism, accuracy, and strong follow-through. The role requires collaboration across departments and the ability to manage deadlines with minimal supervision.
Requirements & Qualifications
Essential Functions
- Assign and sequence ICD-10-CM codes as appropriate.
- Abstract pertinent information from patient records for admissions to Home Care and Hospice, including new and post-hospital referrals.
- Serve as a resource for ICD-10-CM coding issues to support optimal reimbursement in compliance with CMS policies, procedures, and Official Coding Guidelines.
