Medical Coder

Northfield Hospital + Clinics

The Medical Coder reviews medical documentation and assigns accurate diagnostic and procedural codes using ICD-10, CPT, and HCPCS. This role supports billing accuracy, compliance with healthcare regulations, revenue cycle efficiency, and reduction of claim denials. The coder works closely with healthcare providers and billing staff to maintain complete and up-to-date patient records.

Key Accountabilities

  • Review and analyze medical records to identify diagnoses and procedures for accurate coding and reporting.
  • Assign ICD-10, CPT, and HCPCS codes for medical diagnoses and procedures.
  • Ensure coding meets regulatory and payer requirements.
  • Collaborate with providers to clarify ambiguous or incomplete documentation.
  • Monitor coding worklists such as claims, denials, account checks, and tasks to ensure timely resolution.
  • Process and transmit coded medical records to support billing and reimbursement.
  • Participate in internal and external audits to support coding quality.
  • Stay current on coding standards, payer policies, and documentation best practices.
Requirements & Qualifications

Qualifications

  • Completion of a certified medical coding program.
  • AHIMA or AAPC certification.
  • Proficient in speaking, reading, and writing English.
  • Preferred: 6 months of medical coding experience in a clinic or hospital setting.
