1. Indications

The comprehensive audiometry threshold evaluation and speech recognition test (CPT® Code 0212T) is indicated for the following conditions:

  • Hearing Loss Patients presenting with symptoms of hearing loss, which may include difficulty understanding speech, especially in noisy environments, or a noticeable decrease in the ability to hear sounds.
  • Auditory Processing Disorders Individuals suspected of having auditory processing disorders, where the brain has difficulty processing auditory information, despite normal hearing thresholds.
  • Monitoring Hearing Changes Patients requiring ongoing assessment of hearing sensitivity over time, particularly those with known hearing impairments or those undergoing treatment that may affect hearing.
  • Pre-Operative Assessment Individuals undergoing surgical procedures that may impact hearing, necessitating a baseline evaluation of auditory function.

2. Procedure

The procedure for conducting a comprehensive audiometry threshold evaluation and speech recognition test involves several key steps:

  • Step 1: Preparation The patient is positioned comfortably in a sound-treated room to minimize external noise interference. Ambient noise microphones are placed near the patient's ears to monitor the sound environment during testing.
  • Step 2: Ear Insert Placement Ear inserts are carefully placed in the patient's ears. These inserts are designed to fit snugly but do not completely occlude the ear canal, allowing for effective sound transmission while maintaining comfort.
  • Step 3: Connection to Equipment The ear inserts are connected to transducers and a bone oscillator located outside the ear. This setup is essential for conducting both air and bone pure tone audiometry tests.

  • Step 6: Pure Tone Bone Audiometry Following the air audiometry, the bone pure tone audiometry test is performed. The thresholds are also graphically represented on the computer.
  • Step 7: Automated Speech Audiometry The automated speech audiometry component is then conducted, where the patient listens to speech stimuli at varying intensities. The lowest intensity speech stimulus that the patient can hear at least 50 percent of the time is measured and scored by the computer.
  • Step 8: Speech Recognition Testing Finally, the patient participates in a speech recognition test, where they are asked to repeat a set of words presented by the computerized program. The computer calculates the percentage of words repeated correctly, providing valuable data on the patient's speech discrimination abilities.

3. Post-Procedure

After the comprehensive audiometry threshold evaluation and speech recognition test, the patient may be provided with immediate feedback regarding their hearing status. The results, including the thresholds for air and bone conduction as well as speech recognition scores, are typically compiled into a report for review by the healthcare provider. Depending on the findings, further evaluation or intervention may be recommended.
