1. Indications

The application of skin substitute grafts using CPT® Code 15276 is indicated for the treatment of various types of open wounds, particularly in the following scenarios:

  • Burns: Severe skin damage resulting from thermal, chemical, or electrical sources.
  • Skin Donor Sites: Areas where skin has been harvested for grafting purposes, often leaving a wound that requires closure.
  • Diabetic Ulcers: Chronic wounds that develop on the feet or legs of individuals with diabetes due to poor circulation and nerve damage.
  • Venous Ulcers: Open sores that occur due to improper functioning of venous valves, leading to poor blood flow and skin breakdown.
  • Chronic Open Wounds: Wounds that do not heal properly over time, often due to underlying health conditions or inadequate blood supply.

2. Procedure

The procedure for applying a skin substitute graft involves several detailed steps, which may vary depending on the type of skin substitute used. Below are the procedural steps associated with CPT® Code 15276:

  • Preparation of the Wound Bed: The wound area is first cleaned and prepared to ensure optimal conditions for graft application. This may involve debridement to remove any necrotic tissue and to create a healthy base for the graft.
  • Selection of Skin Substitute: The appropriate type of skin substitute is selected based on the specific characteristics of the wound. Options include acellular dermal allografts, tissue cultured allogeneic skin substitutes, tissue cultured allogeneic dermal substitutes, or acellular xenografts.
  • Application of Acellular Dermal Allograft: If an acellular dermal allograft is chosen, the graft sheets are removed from their packaging, rehydrated in an isotonic sodium chloride solution, and trimmed to fit the wound dimensions. The sheets are then applied over the prepared wound bed, either in single or multiple layers, and secured with absorbable sutures. Any excess material at the wound's periphery is trimmed away.
  • Application of Tissue Cultured Allogeneic Skin Substitute: For this type of substitute, the graft is fenestrated, meaning a series of holes or openings are created in the material. This fenestrated skin substitute is then placed on the wound bed and secured with sutures to ensure proper adherence.
  • Application of Tissue Cultured Allogeneic Dermal Substitute: This substitute is applied directly to the wound bed and secured using either sutures or staples, ensuring a stable fit.
  • Application of Acellular Xenograft: The xenograft is cut to the appropriate size and shape to match the wound. It is then applied to the wound bed and secured with sutures to maintain its position.
  • Dressing Application: After the skin substitute is applied, a layered dressing is placed over the graft. This dressing typically includes a nonadherent layer to protect the graft, a bulky layer of gauze for absorption, a compression layer to minimize swelling, and an anti-shear layer to prevent friction.

3. Post-Procedure

59 Distinct procedural service: under certain circumstances, it may be necessary to indicate that a procedure or service was distinct or independent from other non-e/m services performed on the same day. modifier 59 is used to identify procedures/services, other than e/m services, that are not normally reported together, but are appropriate under the circumstances. documentation must support a different session, different procedure or surgery, different site or organ system, separate incision/excision, separate lesion, or separate injury (or area of injury in extensive injuries) not ordinarily encountered or performed on the same day by the same individual. however, when another already established modifier is appropriate it should be used rather than modifier 59. only if no more descriptive modifier is available, and the use of modifier 59 best explains the circumstances, should modifier 59 be used. note: modifier 59 should not be appended to an e/m service. to report a separate and distinct e/m service with a non-e/m service performed on the same date, see modifier 25.
76 Repeat procedure or service by same physician or other qualified health care professional: it may be necessary to indicate that a procedure or service was repeated by the same physician or other qualified health care professional subsequent to the original procedure or service. this circumstance may be reported by adding modifier 76 to the repeated procedure or service. note: this modifier should not be appended to an e/m service.
