CPT Codes For Work Related or Medical Disability Evaluation Services
CPT 99455 Description: CPT 99455 can be reported when the provider evaluates the patient for medical or work-related reasons.
The provider provides this service to established and new patients. The service is provided irrespective of the place of service.
CPT 99456 Description: CPT 99456 can be reported when a provider (other than the patient’s physician) provides a medical disability or work-related exam (such as required for a worker’s compensation claim.)
CPT 99457 Description: CPT 99457 can be reported for the first 20 minutes spent providing this service by a qualified healthcare provider, physician, or clinical staff.
There is interaction with the patient or caregiver to adjust treatment based on the recordings, and the patient’s remote physiologic recordings are monitored and/or programmed alerts.
CPT 99458 Description: CPT 99458 is an add-on code for CPT 99457. CPT 99458 can be reported for every extra 20 minutes spent providing this service by a qualified healthcare provider, physician, or clinical staff.
There is interaction with the patient or caregiver to adjust treatment based on the recordings, and the patient’s remote physiologic recordings are monitored and/or programmed alerts.
