CPT 99483 Description: CPT 99483 can be used when the provider spends 50 minutes face-to-face with the patient, caregiver or family.

The provider thoroughly assesses a cognitively impaired patient’s understanding, memory, comprehension, and reasoning.

The provider reviews the patient’s medication lists (and makes adjustments if needed, and obtains a history. The provider documents and prepares a care plan consistent with the listed required elements that involve medical decision-making (moderately or highly complex).

CPT 99484 Description: CPT 9484 can be reported when 20 minutes (each month) of a staff member’s time is spent on managing and coordinating primary care services and behavioral health for a patient under the direction of a qualified health care professional or physician.

CPT 99485 Description: CPT 99485 may be reported for supervision by a provider (up to 30 minutes) of transporting a child (24 months/2 years or younger) who is critically ill or ill between facilities.

The provider communicates with the transport team before and after transport after. The child has arrived at the receiving facility. The provider interprets the data of the patient relayed by the transport team.

CPT 99486 Description: CPT 99486 is an add-on code for CPT 99485. CPT 99486 may be reported for every additional 30 minutes for the supervision by a provider of the transport of a child (24 months/2 years or younger) who is critically ill between facilities.

The provider communicates with the transport team before and after transport after. The child has arrived at the receiving facility. The provider interprets the data of the patient relayed by the transport team.
