CPT 99375 describes the provision of complex and multidisciplinary care modalities for a patient under the care of a home health agency, including regular development and/or revision of care plans, review of patient status reports, communication with health care professionals, family members, and/or key caregivers, and integration of new information into the medical treatment plan, all within a calendar month for 30 minutes or more.
