Common Language Description

Hypospadias is a congenital condition characterized by the abnormal positioning of the urethral meatus, which is the external opening of the urethra. In males, this opening is typically located at the tip of the penis; however, in hypospadias, it is found on the underside of the penis. The severity of hypospadias can vary, with distal hypospadias being classified as mild. In this case, the meatal opening is small and situated near the glans, or head, of the penis. The procedure described by CPT® Code 54322 involves a one-stage surgical repair of distal hypospadias, which may include the presence of chordee (a curvature of the penis) or circumcision. The surgical technique utilized in this procedure is known as simple meatal advancement, which can be performed using methods such as the MAGPI (meatal advancement and glanuloplasty), inverted Y-meatoglanuloplasty, or V-flap techniques. These methods aim to reposition the urethral opening to its normal location while ensuring the structural integrity and appearance of the penis are preserved. 1. Indications

The procedure described by CPT® Code 54322 is indicated for the surgical correction of distal hypospadias, which is characterized by the following conditions:

  • Distal Hypospadias - A condition where the urethral meatus is located near the glans of the penis, requiring surgical intervention to reposition it.
  • Chordee - The presence of penile curvature that may accompany hypospadias, which can be addressed during the repair.
  • Circumcision - The procedure may be performed in conjunction with circumcision if indicated.

2. Procedure

The surgical procedure for CPT® Code 54322 involves several key steps to effectively repair distal hypospadias:

  • Step 1: Incision - A longitudinal circumcising incision is made in the glans of the penis. This incision is crucial for accessing the urethral meatus and facilitating the advancement of the meatus distally.
  • Step 2: Dorsal Meatotomy - A dorsal meatotomy is performed to allow for the elevation and advancement of the ventral edge of the meatus. This step is essential for repositioning the meatus to its appropriate location.
  • Step 3: Glans Flap Creation - The flattened wings of the glans are rotated upwards and ventrally to form a cone shape. This is achieved by detaching the glans from the corpus spongiosum and the corpora cavernosa at the lateral margins.
  • Step 4: Reapproximation of Glans Tissue - The edges of the glans tissue are reapproximated in the midline around the excised urethral meatus. This is done in two layers: first incorporating the mesenchyme of the glans and then the superficial glans epithelium.
  • Step 5: Alternative Techniques - If an inverted Y-meatoglanuloplasty is performed, a longitudinal incision is made midline from the meatus to the edge of the glans, with diverging limb incisions creating space for the new urethra. The Y-incision is sutured as an inverted V, and the glans flaps are wrapped around the urethra and sutured in the midline.

3. Post-Procedure

Following the procedure, patients are typically monitored for any immediate complications. Post-operative care may include the placement of a urethral stent or catheter to ensure proper drainage and healing of the newly formed urethra. Patients are advised on care instructions to maintain hygiene and monitor for signs of infection or complications. The expected recovery period may vary, but patients generally experience a gradual return to normal activities as healing progresses. Follow-up appointments are essential to assess the surgical outcome and ensure the proper function of the urethra.
