Official Description

Common Language Description

Hypospadias is a congenital condition characterized by the abnormal positioning of the urethral meatus, which is the opening through which urine exits the body. In males with hypospadias, this opening is located on the underside of the penis rather than at the tip, which is the normal anatomical position. The condition is classified into various types based on the location of the meatus, with distal hypospadias being the mildest form. In distal hypospadias, the meatal opening is situated near the glans or the tip of the penis, making it less severe compared to more proximal forms of hypospadias. The surgical procedure coded as CPT® 54324 involves a one-stage repair of distal hypospadias, which may include the presence of chordee (a curvature of the penis) or circumcision. This procedure is performed using local skin flaps for urethroplasty, which is the surgical reconstruction of the urethra. Techniques such as the Mathieu or tubularized incised plate (TIP) urethroplasty, commonly known as the Snodgrass procedure, are utilized to create a new urethra. The goal of this surgical intervention is to restore normal urinary function and improve the cosmetic appearance of the penis, thereby addressing both functional and aesthetic concerns associated with hypospadias.

1. Indications

The procedure coded as CPT® 54324 is indicated for the surgical correction of distal hypospadias in males. The specific indications for performing this procedure include:

  • Distal Hypospadias - The presence of a urethral meatus located near the glans or tip of the penis.
  • Chordee - The presence of penile curvature that may accompany the hypospadias condition.
  • Circumcision - The procedure may be performed in conjunction with circumcision if indicated.

2. Procedure

The surgical procedure for CPT® 54324 involves several detailed steps to achieve a successful one-stage distal hypospadias repair with urethroplasty using local skin flaps. The following procedural steps are performed:

  • Step 1: Incision and Preparation - A U-shaped incision is made along the urethral plate, and the penis is degloved to expose the underlying structures. This initial step is crucial for accessing the urethral plate and preparing for the reconstruction.
  • Step 2: Creating Glans Wings - Longitudinal incisions are made along the urethral plate to create the glans wings, which will play a significant role in the reconstruction of the urethra.
  • Step 3: Flap Development - In the Mathieu procedure, a skin flap is developed proximal to the urethral meatus, ensuring it has sufficient length to reach the tip of the glans. This flap is then elevated and sutured to each side of the distal urethra, facilitating the formation of a new urethral channel.
  • Step 4: Urethral Reconstruction - In the Snodgrass procedure, a midline incision is made from the tip of the glans to the urethral meatus, extending through the mucosal and submucosal tissue down to the corporal bodies. This creates a tubular structure over a catheter, which is essential for the new urethra.
  • Step 5: Tissue Rotation - The dorsal subcutaneous tissue from the preputial and penile shaft skin is dissected and rotated ventrally to cover the neo-urethra. This step is vital for providing adequate support and coverage for the newly formed urethra.
  • Step 6: Closure - The wings of the glans are closed symmetrically in two layers, providing additional support to the neo-urethra. This layered closure technique is important for ensuring the integrity and functionality of the reconstructed urethra.
  • Step 7: Stenting - A urethral stent or catheter is placed and remains in position following the procedure to facilitate healing and maintain the patency of the newly constructed urethra.

3. Post-Procedure

After the completion of the procedure coded as CPT® 54324, post-operative care is essential for optimal recovery. Patients are typically monitored for any signs of complications, such as infection or bleeding. The urethral stent or catheter that is placed during the procedure will remain in situ for a specified duration to ensure proper healing of the neo-urethra. Patients may be advised on activity restrictions to prevent undue stress on the surgical site. Follow-up appointments are crucial to assess the healing process and the functionality of the reconstructed urethra. Any concerns regarding urinary function or cosmetic appearance should be addressed during these follow-up visits to ensure the best possible outcomes for the patient.
