Official Description

1-stage distal hypospadias repair (with or without chordee or circumcision); with urethroplasty by local skin flaps and mobilization of urethra

Common Language Description

Hypospadias is a congenital condition characterized by the abnormal positioning of the urethral meatus, which is the opening through which urine exits the body. In males with hypospadias, this opening is located on the underside of the penis rather than at the tip, which is the normal anatomical position. The condition is classified into various types based on the location of the meatus, with distal hypospadias being the mildest form. In distal hypospadias, the meatal opening is situated near the glans, the rounded tip of the penis. The surgical procedure coded as CPT® 54326 involves a one-stage repair of distal hypospadias, which may be performed with or without the presence of chordee (a curvature of the penis) or circumcision. This procedure includes urethroplasty, which is the surgical reconstruction of the urethra, utilizing local skin flaps and the mobilization of the urethra itself. The mobilization process entails detaching the urethra from the anterior aspect of the corpora cavernosa, which are the erectile tissues of the penis, to enhance the length of the urethra. This technique is sometimes referred to as Koff's technique, and if additional length is necessary, the urethra may also be freed proximally, a method known as the Turner-Warwick procedure. The goal of this surgical intervention is to restore normal urinary function and improve the cosmetic appearance of the penis.

1. Indications

The procedure coded as CPT® 54326 is indicated for the surgical correction of distal hypospadias in males. This condition is characterized by the abnormal placement of the urethral meatus on the underside of the penis, which can lead to various functional and cosmetic issues. The indications for performing this one-stage repair may include:

  • Distal Hypospadias - The primary indication for this procedure is the presence of distal hypospadias, where the meatus is located near the glans.
  • Chordee - If present, chordee, or the curvature of the penis, may also necessitate surgical intervention to ensure proper alignment and function.
  • Cosmetic Concerns - Patients or guardians may seek surgical correction for aesthetic reasons, as the abnormal positioning of the meatus can affect the appearance of the penis.
  • Urinary Function Issues - The procedure may be indicated if the abnormal meatal position leads to difficulties with urination or other urinary tract complications.

2. Procedure

The procedure for CPT® 54326 involves several detailed steps to effectively repair distal hypospadias. The following procedural steps are typically performed:

  • Step 1: Anesthesia and Preparation - The patient is placed under appropriate anesthesia, and the surgical area is prepared to maintain a sterile environment.
  • Step 2: Incision and Exposure - A longitudinal incision is made to access the urethra and surrounding tissues. This incision allows for the mobilization of the urethra and the creation of local skin flaps.
  • Step 3: Mobilization of the Urethra - The urethra is carefully detached from the anterior aspect of the corpora cavernosa. This mobilization is crucial for increasing the length of the urethra, which may involve the use of Koff's technique.
  • Step 4: Urethroplasty - Local skin flaps are utilized to reconstruct the urethra. The flaps are shaped and sutured to form a new urethral channel, ensuring proper alignment and function.
  • Step 5: Closure of the Incision - Once the urethroplasty is completed, the incision is closed in layers to promote healing and minimize scarring. The glans wings may also be closed symmetrically to provide additional support to the neo-urethra.
  • Step 6: Placement of Stent or Catheter - A urethral stent or catheter is placed to maintain patency of the newly formed urethra during the initial healing phase.

3. Post-Procedure

After the completion of the procedure coded as CPT® 54326, specific post-operative care is essential for optimal recovery. Patients are typically monitored for any signs of complications, such as infection or bleeding. The urethral stent or catheter remains in place for a specified duration to ensure that the newly constructed urethra remains open and functional during the healing process. Patients may be advised to avoid strenuous activities and to follow specific guidelines regarding hygiene and care of the surgical site. Follow-up appointments are crucial to assess healing, remove the catheter or stent, and evaluate the success of the procedure in restoring normal urinary function and appearance.
