Common Language Description

A semen analysis is a laboratory test that assesses the characteristics of a man's semen, specifically focusing on the presence and motility of sperm. This analysis is crucial for evaluating male fertility. The procedure involves measuring the total sperm count, which is the number of sperm present in a milliliter of semen. A normal sperm count is defined as 20 million sperm per milliliter or more. Additionally, the motility of the sperm is evaluated, which refers to the ability of sperm to move actively and effectively. This is important because motile sperm are necessary for successful fertilization, as they must travel from the site of deposition to the egg for conception to occur. A normal motility count is considered to be 8 million motile sperm per milliliter or higher. It is important to note that this specific CPT® code, 89310, is used when assessing motility and count, but does not include the Huhner test. For a more comprehensive analysis that includes volume and differential evaluation, the CPT® code 89320 should be utilized. The volume assessment measures the total amount of semen produced in a single ejaculation, while the differential evaluation examines the morphology of sperm, determining how many appear normal under microscopic observation.

1. Indications

The semen analysis is indicated for various reasons related to male reproductive health. The following conditions may warrant this procedure:

  • Infertility Evaluation This test is commonly performed as part of the infertility workup for couples trying to conceive, particularly when there are concerns regarding male fertility.
  • Semen Quality Assessment It is used to assess the quality of semen in men who have undergone vasectomy reversal or other surgical procedures affecting fertility.
  • Hormonal Imbalance Investigation The analysis may be indicated when there are signs of hormonal imbalances that could affect sperm production and motility.
  • Post-Surgical Monitoring It can be performed to monitor sperm health after surgeries such as varicocele repair or other interventions that may impact sperm production.

2. Procedure

The semen analysis procedure involves several key steps to ensure accurate results. The following outlines the procedural steps:

  • Step 1: Sample Collection The patient is instructed to provide a semen sample, typically through masturbation, into a sterile container. It is important that the sample is collected after a period of sexual abstinence, usually 2 to 7 days, to ensure optimal sperm concentration.
  • Step 2: Sample Transport Once collected, the semen sample must be transported to the laboratory promptly, ideally within one hour, to maintain the viability of the sperm for accurate analysis.
  • Step 3: Analysis of Sperm Count In the laboratory, the total sperm count is determined by counting the number of sperm present in a specified volume of semen, with a focus on achieving a count of 20 million sperm per milliliter or higher for normal results.
  • Step 4: Assessment of Motility The motility of the sperm is then evaluated, where the number of actively moving sperm is counted. A normal motility count is defined as 8 million motile sperm per milliliter or more, indicating good forward motion necessary for fertilization.

3. Post-Procedure

After the semen analysis is completed, the results are typically reviewed by a healthcare provider. The patient may be advised on the next steps based on the findings, especially if abnormalities are detected in sperm count or motility. If the results indicate potential fertility issues, further testing or referrals to a specialist may be recommended. It is also important for patients to discuss any lifestyle factors that could affect sperm health, such as smoking, alcohol consumption, and diet, as these can play a significant role in fertility outcomes.
