1. HCPCS Code M1019

HCPCS code M1019 is used to indicate that an adolescent patient aged 12 to 17 years with major depression or dysthymia has reached remission at twelve months, as demonstrated by a twelve month (+/-60 days) PHQ-9 or PHQ-9M score of less than 5.

2. HCPCS Code M1020

HCPCS code M1020 is used to indicate that an adolescent patient aged 12 to 17 years with major depression or dysthymia has not reached remission at twelve months, as demonstrated by a twelve month (+/-60 days) PHQ-9 or PHQ-9M score of less than 5. This code is also used when the PHQ-9 or PHQ-9M score was not assessed or is greater than or equal to 5.

3. HCPCS Code M1021

HCPCS code M1021 is used to indicate that the patient had only urgent care visits during the performance period.

4. HCPCS Code M1022

HCPCS code M1022 is used to indicate that the patient had at least one outpatient visit with a mental health professional during the performance period.

5. HCPCS Code M1023

HCPCS code M1023 is used to indicate that the patient had at least one inpatient visit during the performance period.

6. HCPCS Code M1024

HCPCS code M1024 is used to indicate that the patient had at least one emergency department visit during the performance period.

7. HCPCS Code M1025

HCPCS code M1025 is used to indicate that the patient had at least one telehealth visit during the performance period.

8. HCPCS Code M1026

HCPCS code M1026 is used to indicate that the patient had at least one medication management visit during the performance period.
