HCPCS G2003 describes a moderate (45 minutes) in-home visit for a new patient post-discharge, within 90 days following discharge from an inpatient facility and no more than 9 times.

4. HCPCS Code G2004

HCPCS G2004 describes a comprehensive (60 minutes) in-home visit for a new patient post-discharge, within 90 days following discharge from an inpatient facility and no more than 9 times.

5. HCPCS Code G2005

HCPCS G2005 describes an extensive (75 minutes) in-home visit for a new patient post-discharge, within 90 days following discharge from an inpatient facility and no more than 9 times.

6. HCPCS Code G2006

HCPCS G2006 describes a brief (20 minutes) in-home visit for an existing patient post-discharge, within 90 days following discharge from an inpatient facility and no more than 9 times.

7. HCPCS Code G2007

HCPCS G2007 describes a limited (30 minutes) in-home visit for an existing patient post-discharge, within 90 days following discharge from an inpatient facility and no more than 9 times.

8. HCPCS Code G2008

HCPCS G2008 describes a moderate (45 minutes) in-home visit for an existing patient post-discharge, within 90 days following discharge from an inpatient facility and no more than 9 times.

9. HCPCS Code G2009

HCPCS G2009 describes a comprehensive (60 minutes) in-home visit for an existing patient post-discharge, within 90 days following discharge from an inpatient facility and no more than 9 times.

10. HCPCS Code G2010

HCPCS G2010 describes the remote evaluation of recorded video and/or images submitted by an established patient, including interpretation and follow-up within 24 business hours, not originating from a related E/M service provided within the previous 7 days nor leading to an E/M service or procedure within the next 24 hours or soonest available appointment.

11. HCPCS Code G2011

HCPCS G2011 describes an alcohol and/or substance misuse structured assessment and brief intervention, lasting 5-14 minutes.

12. HCPCS Code G2012

HCPCS G2012 describes a brief communication technology-based service, such as virtual check-in, provided by a physician or other qualified healthcare professional to an established patient, not originating from a related E/M service provided within the previous 7 days nor leading to an E/M service or procedure within the next 24 hours or soonest available appointment, with 5-10 minutes of medical discussion.

13. HCPCS Code G2013

HCPCS G2013 describes an extensive (75 minutes) in-home visit for an existing patient post-discharge, within 90 days following discharge from an inpatient facility and no more than 9 times.

14. HCPCS Code G2014

HCPCS G2014 describes limited (30 minutes) care plan oversight, within 90 days following discharge from an inpatient facility and no more than 9 times.

15. HCPCS Code G2015

HCPCS G2015 describes comprehensive (60 minutes) home care plan oversight, within 90 days following discharge from an inpatient facility.

16. HCPCS Code G2020

HCPCS G2020 describes services for high intensity clinical services associated with the initial engagement and outreach of beneficiaries assigned to the SIP component of the PCF model.
