An established patient is is a patient that is seen by a physician (in the group practise or a physician that bills under the same group number) in the past 36 months for a face to face service.
To support the service level, at least two of three key components must be met.
- Component 1: A problem focused examination.
- Component 2: A problem focused interval history.
- Component 3: Medical decision making that is straightforward or of low complexity.
