CPT 99374 Description: CPT 99374 may be used when a provider spends 15 to 29 minutes on the indirect care of a patient. CPT 99374 can be used when the provider oversees the plan of care (a home health agency provides that). CPT 99374 can only be used while the patient is in a facility meant for long-term care or at home.

CPT 99374 can be used when the provider oversees the plan of care (a home health agency provides that). CPT 99377 Description: CPT 99377 can be used when a provider spends 15 to 29 minutes on the patient’s indirect care. CPT 99377 can be reported when the care is coordinated and supervised by the provider and provided to a hospice patient.
CPT 99378 Description: CPT 99378 can be used when a provider spends 30+ minutes on the patient’s indirect care. CPT 99378 can be reported when the care is coordinated and supervised by the provider and provided to a hospice patient.
CPT 99379 Description: CPT 99379 can be reported when the provider spends 15 to 29 minutes on the patient’s indirect care. CPT 99379 may be reported the provider supervises the care provided at a nursing facility to oversee the care plan.
CPT 99380 Description: CPT 99380 can be reported when the provider spends 30+ minutes on the patient’s indirect care. CPT 99380 may be reported the provider supervises the care provided at a nursing facility to oversee the care plan.
