Description Of Well Woman Exam
HMSA plans require every woman to have at least one gynecological exam per calendar year. This can include a pelvic exam, a clinical breast exam, taking a sample and / or a Pap smear when screening is carried out.
A well-woman exam is billed when a patient sees a doctor for a routine annual gynaecological exam. If a patient has gynaecological symptoms (e.g. Dysmenorrhea), the doctor will evaluate and administer the E & M visit, which will be billed directly.
Do not count on laboratory services associated with a Pap smear unless you have a CLIA-approved lab in your office that provides these services.
If the sample is sent to an external laboratory, this laboratory will be accounted for under HMSA.
Modifier For Well Woman
Append the modifier 25 to the appropriate office E&M CPT 99201 – CPT 99215. When a patient is seen for a physical and a screening Pap smear is collected at the time, code the E&M and Pap collection HCPCS code Q0091 (Screening Papanicolaou smear; obtaining, preparing and conveyance of cervical or vaginal smear to laboratory).
